docs: add debug commands to projman documentation

- Add /debug-report and /debug-review to projman README
- Add debug commands to COMMANDS-CHEATSHEET.md
- Remove obsolete doc-guardian Stop hook references from cheatsheet
- Update Architecture section with debug command files
