feat(netbox)!: gut server from 182 to 37 tools

BREAKING CHANGE: Removed circuits, tenancy, VPN, wireless modules entirely.
Stripped DCIM, IPAM, virtualization, extras to only essential tools.
Deleted NETBOX_ENABLED_MODULES filtering — no longer needed.
- Delete circuits.py, tenancy.py, vpn.py, wireless.py
- Strip dcim.py to sites, devices, interfaces only (11 tools)
- Strip ipam.py to IPs, prefixes, services only (10 tools)
- Strip virtualization.py to clusters, VMs, VM interfaces only (10 tools)
- Strip extras.py to tags, journal entries only (6 tools)
- Remove all module filtering code from config.py and server.py
- Rewrite README.md with accurate 37-tool documentation
- Update CHANGELOG.md with breaking change entry
- Token reduction: ~19,810 → ~3,700 (~81%)
Remaining work: Update cmdb-assistant plugin skills (10 files) in follow-up PR

feat(netbox): add module-based tool filtering for token optimization

Reduces NetBox MCP context token consumption from ~19,810 tokens (182 tools)
to ~4,500 tokens (~43 tools) by enabling environment-variable-driven module
filtering.
Key changes:
- Add NETBOX_ENABLED_MODULES env var to config.py
- Filter tool registration based on enabled modules in server.py
- Conditional tool class instantiation for memory efficiency
- Routing guard with clear error messages for disabled modules
- Startup logging shows enabled modules and tool count
Also fixes documentation referencing incorrect tool names:
- virtualization_* → virt_* in cmdb-assistant docs
- wireless_* → wlan_* in README
- circuits_list_circuit_terminations → circ_list_terminations
Recommended config for cmdb-assistant users:
NETBOX_ENABLED_MODULES=dcim,ipam,virtualization,extras

refactor: bundle MCP servers inside plugins for cache compatibility

Claude Code only caches the plugin directory when installed from a
marketplace, not parent directories. This broke the shared mcp-servers/
architecture because relative paths like ../../mcp-servers/ resolved
to non-existent locations in the cache.
Changes:
- Move gitea and wikijs MCP servers into plugins/projman/mcp-servers/
- Move netbox MCP server into plugins/cmdb-assistant/mcp-servers/
- Update .mcp.json files to use ${CLAUDE_PLUGIN_ROOT}/mcp-servers/
- Update setup.sh to handle new bundled structure
- Add netbox.env config template to setup.sh
- Update CLAUDE.md and CANONICAL-PATHS.md documentation
This ensures plugins work correctly when installed and cached.
